Health Action Council Kick-Off Conference 2010 January 28, 2010 - Cleveland Marriott Downtown at Key Center Register at hacohio.org All attendees must register and remit payment in full prior to the conference. Payment may be made by check (payable to Health Action Council Ohio) or by credit card using PayPal. For either conference, please indicate your registration type: HAC Employer Members (Regular, Public & Associate) No Charge HAC Sponsor Members $199 General Admission • Employers that are not HAC members • Hospitals (Leapfrog reporting hospitals can send up to two attendees at no charge.) • Vendors & all other non-HAC members $239 Registration & Exhibit Hall open at 7:30 AM. Full day conference includes breakfast, lunch and reception. No one will be admitted without registration. Name tags must be displayed at all times. C
