The Dr. Amy L. Bernard Memorial Scholarship

Application Information

The application period is open from July 15 to September 25, 2026.  Applicants will be notified of a decision on or before October 13, 2025, at the 2025 Health Educator's Institute. 

Eligibility Requirements for Applicants 

• Must be a full-time or part-time student for the 2026-2027 academic year; and
• Must be enrolled in a health education/promotion or related program; and
• Must be a current resident of Ohio; and
• Must plan to work in a community or public health setting; and 
• Must have a GPA of 3.0 or greater.

Timeline  

• Application period is from July 15 to September 25. The deadline to apply is September 25 at 11:59 p.m.

• Applicants will need to complete the application form and submit a letter of recommendation from a faculty member AND provide unofficial/official transcripts.

3. Applicants will email to a SharePoint Folder to upload your unofficial/official transcripts and a letter of recommendation from a faculty member.

• Each applicant will receive an email containing a unique SharePoint folder link where they can upload their required documents.
