Premier Community Health in Dayton, Ohio has an opening for a Population Health Data Manager.
This position reports to the Executive Director and is an exempt position. This individual will supervise one part-time data entry specialist.
Position Summary The Population Health Data Manager is responsible for management and oversight of all data collection systems, data reporting and data integrity. He/she works with groups we serve with data needs and “translates” data into useable information. He/she also has primary responsibility for the management of the Applied Health Analytics program, including updating internal and external vendor information, working with employers to assure easy access, and working with Applied Health Analytics. He/she also works with staff to ensure all policies and procedures are up to date in program areas, explores new programming opportunities and assesses current programming. When appropriate, he/she works with PHP to ensure data continuity and relevance. This position has one direct report- a part-time data entry specialist.
Strategic Relevance This contributes to PCH’s mission because it enables us to better measure and report on programmatic outcomes and progress and ensure our programs are evidence-based and relevant. This position is also key in assuring the quality of our employer programs and ensuring employers have optimal ease of use with access to understandable data.
Principal Duties and Responsibilities · Management of all aspects of the organizational programmatic database. Ensures data is up to date and accurate. · Produces programmatic reports by assigned deadlines · Works with Applied Health Analytics as the primary organizational liaison. · Ensures information in AHA database is up to date and accurate. · Customizes AHA database for groups, as needed · Recruits internal and external vendors for AHA database. Maintains communications/relationships with these vendors. · Creates plan for sending relationship/referral e-mails to participants in the HAA database. · Works with AHA groups to assure we meet their data needs and they understand the data that is available to them. · Assures standards and policies that are relevant to programmatic areas are up to date and meet evidence guidelines and all appropriate federal, state and local laws and/or industry regulations · Explores possible new programming areas. Researches scholarly literature and evidence base, clinical guidelines, etc. for proposals. · Assesses current programs to determine if they are meeting the intended goals. Revising outcomes goals and objectives in collaboration with program staff, as needed. Recommending programs for continuation, update or termination. · Works with contacts at PHP whenever a data issue overlaps with PHP. · Manages part-time data entry specialist · Assists with grant proposal preparation. · Works on special projects, as assigned.
Job Qualifications Undergraduate degree in health promotion/community health or related field required. MPH or MS preferred. CHES preferred. Must have excellent understanding of databases and how they work. Must also understand data issues in relationship to community health education. Must work well with individual at all levels or organizations, including "C" level. Must understand basic community health concepts and theories. Must have thorough understanding of evaluation and outcome measurement. At least one year of relevant experience. Should be proficient in Microsoft Word, Excel, PowerPoint and Outlook.

Physical Requirements & Working Conditions Normal office environment – incumbent must possess all visual skills. PCH is a tobacco and fragrance-free environment. There is no tobacco use on the campus, in company cars or trucks or in personal vehicles being used for company business with another passenger in the car. Because position requires working in the community, an active Ohio driver’s license and reliable transportation are required.
